Incorrect installation of SPD's is a regular occurrence, usually repeated throughout the building. A typical secondary SPD installed at a Distribution Board without an MEN requires the phases to be connected to Neutral via the SPD and the Neutral to be connected to Earth using an integrated gas discharge tube. Check out Figure F2, Appendix F AS3000:2018. Selection of correct equipment for the application is everything....

Old components fail over time. Their lifetime can be shortened by cramming them in like sardines. Make sure the components in your switchboard are sized, positioned, spaced, ventilated and not operating well beyond their expected lifetime.
